Submission of a Booking Form constitutes acceptance of these terms and your continued use of our services confirms your acceptance of the terms in force each time you use Wagging Tails. I hereby confirm that I am the owner/s of the above mentioned dog/s and that I authorise Wagging Tails and their Carers to act as guardian during my absence and to take any action they consider necessary to protect the health and welfare of the above mentioned dog/s.

I also confirm that the information supplied is correct and that any additional information that Wagging Tails should be aware of is also supplied and Wagging Tails will be made aware of any changes.

I give Wagging Tails and/or their Carers permission to transport my dog to the above-mentioned vet or the closest vet to the Carer boarding my dog/s. I understand that Wagging Tails and the Carer assumes no responsibility for the loss of the dog and is released from any liability related to transportation, treatment and costs for which I am fully responsible. I agree to reimburse in full the detailed costs upon collection of my dog.
